NOTICE OF UNDERGROUND STORAGE TANK (UST) CORRECTIVE ACTION PLAN
AND REQUEST FOR COMMENTS

Chevron Environmental Management Company, 1500 Louisiana Street, Mail Box 38154, Houston, Texas, 77002 is required by ADEM to implement a Corrective Action Plan for remediation of soil and/or groundwater which has resulted from a release from an underground storage tank located at the former Chevron 45480, 101 U.S. Hwy. 231, Ozark, Alabama.  The site is identified as Facility I.D. Number 10950-045-005241 and Incident Number UST93-05-33.  The proposed remediation method is injection of chemical oxidants.

Copies of the Corrective Action Plan for the former Chevron 45480 are available for public inspection electronically via http://adem.alabama.gov/newsEvents/publicNotices.cnt and at the following location Monday – Friday (except legal holidays), 8:00 am to 5:00 pm.  A nominal fee for copying and/or mailing may be charged.  Arrangements for copying should be made in advance.

Persons wishing to comment may do so, in writing, to the Department's named contact above within 15 days following the publication date of this notice.  Any person wishing to provide additional information regarding the Corrective Action Plan may do so.

A written request for a public meeting may also be filed within that 15-day period and must state the nature of the issues proposed to be raised in the meeting.
