National Pollutant Discharge Elimination System
(NPDES) Forms
Must accompany the EPA Form 1, 2C, 2D, 2E or 2F. (161 KB file

Is used in conjunction with ADEM Form 187, EPA Form 1, 2C, 2D, 2E and
2F.
Is used for renewal of processed wastewater NPDES industrial permits and must be accompanied
by ADEM Form 187 and EPA Form 1.
Is used for initial applications of NPDES industrial permits and must be
accompanied by ADEM Form 187 and EPA Form 1.
Is used for facilities which do not discharge process wastewater.
This form is generally used for non-contact water. ADEM Form 187 and EPA
Form 1 must accompany this form.
Is used for all discharges of storm water associated with an
industrial activity. Facilities whose discharges are composed of storm
water and non-storm water must submit the appropriate non-storm water
form and Form 2F for their storm water. ADEM Form 187 and EPA Form 1 must
accompany this form.

Required for discharges to Municipal or other treatment works owned and

ALG360000: Hydropower
This application should be
completed by new hydroelectric facilities seeking coverage on or after
10/01/2004. This application should also be completed by existing
hydroelectric facilities that have experienced operational changes and are
seeking reissuance.

ADEM Form 395
Authorizes the discharge of minor discharges from hydropower facilities
and imposes best management practices on construction, repair and
maintenance activities conducted above water. (165KB file in Microsoft Word

Authorizes the discharge of filter
backwash water, sedimentation basin washdown, and decant water from water
treatment plants. (76KB file in

Authorizes the discharge of hydrostatic test water. (90KB file in

If a facility now has a “condition of no
exposure,” then the facility is excluded from NPDES storm water permitting.
The Storm Water Phase II Final Rule which was signed on October 29, 1999 has
a “no exposure” exemption clause. Any non-storm water discharges would of
course require proper permitting. If the facility does not have any
industrial exposure and does not have any non-storm water discharges , then
it should do the following:
Complete the attached EPA No Exposure Certification Form in its
entirety. Have a corporate official of the level of at least
vice president sign the form. Return the completed EPA Form to:
