• Notice of Registration (NOR) for Construction equal to or greater than 1 acre and Noncoal Mining/Dry Processing Less than 5 Acres in Size    ADEM Form 498
  
  Notice Of Registration (NOR) form to apply for coverage under the National Pollutant Discharge Elimination System (NPDES) Program (ADEM Admin. Code Chapter 335-6-12) for stormwater discharges from construction, excavation reclamation, land clearing, other regulated land disturbance activities equal to or greater than 1 acre in size, and noncoal mining and/or noncoal, nonmetallic, dry mineral processing sites less than 5 acres in size.

  Request for termination of registration for regulated construction, and noncoal mining/dry processing sites less than 5 acres in size which can be submitted when all construction or regulated disturbance is complete and full reclamation and/or effective site stormwater quality remediation is achieved.
